Submit Section 6 completed by Authorized Medical Examiner.
Situation 2 - If you participated in a fall school sport this school year and NO medical treatment was required from a licensed physician of medicine or osteopathic medicine since submitting physical for that sport . . .
Submit Section 7 completed by parent.
Situation 3 - If you played a fall sport and required medical treatment from a LICENSED PHYSICIAN OF MEDICINE OR OSTEOPATHIC MEDICINE . . .
Submit Section 8 completed by LICENSED PHYSICIAN OF MEDICINE OR OSTEOPATHIC MEDICINE
All Situations . . .
Do not complete/submit Section 9; it does not apply to swimming.
