PIAA Physical Form

Instructions:

Situation 1 - If swimming is your first school sport of the school year:

• Submit Sections 1 through 5 completed by parent.

• Submit Section 6 completed by Authorized Medical Examiner.

Situation 2 - If you participated in a fall school sport this school year and NO medical treatment was required from a licensed physician of medicine or osteopathic medicine since submitting physical for that sport:

• Submit Section 7 completed by parent. 

Situation 3 - If you played a fall sport and required medical treatment from a LICENSED PHYSICIAN OF MEDICINE OR OSTEOPATHIC MEDICINE:

• Submit Section 8 completed by LICENSED PHYSICIAN OF MEDICINE OR OSTEOPATHIC MEDICINE

All Situations:

• Do not complete/submit Section 9; it does not apply to swimming.
